Application forms for Combined Pre-Medical Test (CPMT)
2013 shall be available in branches of Central Bank of India (CBI).
According to the details provided by the
administration of Chattrapati Shahuji Maharaj University, Kanpur, responsible
for conducting the CPMT 2013, the application forms shall be made available in
141 branches of Central Bank of India from April 25, 2013.

Forms can be obtained in various cities of Uttar Pradesh, Uttarakhand and Delhi on cash payment at CBI branches. The application fee is `1280 for General/OBC candidates and `680 for SC/ST candidates.
Application forms shall be available till May 8,
2013. Last date for submission of application forms is May 15, 2013.
