• Pharmacist (Allopathic) -- Pay Band -1 `5200-20200 + Grade Pay `2800

• Payment of application fee can be done in any branch of Punjab National Bank on the bank challan form downloaded from the website.
• The application fee is `125. Fee exemption for SC/ST/PWD/departmental candidates/female candidates and ex-servicemen.
• After applying online, candidates have to send the system generated application form with duly supported attested copies of the relevant certificates and marks statements (in English or Hindi) along with Bank Challan by Registered post /Speed post to the following address – The Joint Director (Admin.), Employees State Insurance Corporation Directorate (Medical Delhi, ESI Scheme: Dispensary Complex, Tilak Vihar, (Tilak Nagar), New Delhi – 110018.
• Documents to be attached along with the application form are -- date of birth certificate, all educational qualification (along with marks sheet), technical/professional qualification along with mark sheets, registration certificate with the concerned council wherever required, caste certificate wherever required, experience certificate wherever required, completion of internship certificate wherever required, disability certificate in case of candidate with disabilities (physically challenged), discharge certificate for ex-serviceman, copy of challan/online payment receipt in support of payment of fee wherever required.
