per the orders of the Mission Director, Rashtriya Gramin Swasthya Mission -
Uttar Pradesh (National Rural Health Mission Uttar Pradesh), Lucknow, the
Office of the Chief Medical Officer, Etah invites applications for the posts of
Medical Officer, Staff Nurse and ANM on purely contract basis for various
projects of the NRHM.
Position
and vacancies:
- Medical Officer – M.B.B.S. – 1 post in the Urban Health Post Project
- Staff Nurse (Female) – 6 posts in the Ashirvaad Baal Swasthya Guarantee Yojna
- ANM (Female) – 3 posts in the C.H.C., Aliganj and P.H.C., Jaithra under the NRHM
Eligibility:
- Medical Officer – M.B.B.S. and registered with the U.P. Faculty.
- Staff Nurse – Diploma in J.N.M. and registered with the U.P. Medical Faculty
- ANM – Diploma in A.N.M and registered with the U.P. Medical Faculty
Age
limit: Between
21 to 65 years of age as on December 31, 2012

- Application along with attested copies of all the relevant documents should be sent by registered post/ speed post to the following address – The Office of the Chief Medical Officer, Etah.
- Mention the post and concerned project name on the envelope containing the application form.
- The details to be included in the application are – passport size photograph, advertisement number, post applied for, name of the applicant, father’s/husband’s name, date of birth and age as on August 31, 2012, category, permanent address, address for correspondence, contact number, mobile number, email id, educational qualifications, work experience, domicile, declaration, signature and total no. of enclosures.
