- Payment of application fee can be done in any branch of Punjab National Bank on the bank challan form downloaded from the website.
- The application fee is `75 for grade pay up to `2000 and `125 for grade pay above `2001. Fee exemption for SC/ST/PWD/departmental candidates/female candidates and ex-servicemen.
- After applying online, candidates have to send the system generated application form with duly supported attested copies of the relevant certificates and marks statements (in English or Hindi) along with Bank Challan in an envelope super scribed -- ‘Application for the post of ___, post code ___, state ___’
- Send the complete application form by Registered post /Speed post at the following address – The Deputy Director (Admn.), Employees' State Insurance Corporation, ESIC Model Hospital Haryana, Sector 9A, Gurgaon -122001, Haryana.
- Documents to be attached along with the application form are -- date of birth certificate, all educational qualification (along with marks sheet), technical/professional qualification along with mark sheets, registration certificate with the concerned council wherever required, caste certificate wherever required, experience certificate wherever required, completion of internship certificate wherever required, disability certificate in case of candidate with disabilities (physically challenged), discharge certificate for ex-serviceman, copy of challan/online payment receipt in support of payment of fee wherever required.
