- Staff Nurse/Dietician -- Pay Band 2 `9300-34800 + Grade Pay `4600
- Occupational Therapist -- Pay Band 2 `9300-34800 + Grade Pay `4200
- Pharmacist (Allopathy) -- Pay Band -1 `5200-20200 + Grade Pay `2800
- O.T. Technician/ECG Technician/Social Worker -- Pay Band -1 `5200-20200 + Grade Pay `2400
- Operation Theatre Assistant/ Laboratory Assistant/Jr. Radiographer -- Pay Band -1 `5200-20200 + Grade Pay `2000

- Payment of application fee can be done in any branch of Punjab National Bank on the bank challan form downloaded from the website.
- The application fee is `125 (plus `25 as bank collection charges) for Grade Pay `1800 to 2400 and `225 (plus `25 as bank collection charges) for Grade Pay `2401 and above . Fee exemption for SC/ST/PWD/departmental candidates/female candidates and ex-servicemen.
- After applying online, candidates have to send the system generated application form with duly supported attested copies of the relevant certificates and marks statements along with Bank Challan by Registered post /Speed post to the following address – ESIC Model Hospital, Industrial Area Phase-II, Ramdarbar, Chandigarh 160002.
- Documents to be attached along with the application form are -- date of birth certificate, all educational qualification (along with marks sheet), technical/professional qualification along with mark sheets, registration certificate with the concerned council wherever required, caste certificate wherever required, experience certificate wherever required, completion of internship certificate wherever required, disability certificate in case of candidate with disabilities (physically challenged), discharge certificate for ex-serviceman, copy of challan/online payment receipt in support of payment of fee wherever required.
