• Payment of application fee can be done in any branch of Punjab National Bank on the bank challan form downloaded from the website.
• The application fee is `125 (bank collection charges extra) for Grade Pay `1800 to 2000 and `225 (bank collection charges extra) for Grade Pay `2001 and above . Fee exemption for SC/ST/PWD/departmental candidates/female candidates and ex-servicemen.
• After applying online, candidates have to send the system generated application form with duly supported attested copies of the relevant documents along with Bank Challan in an envelope super scribed  -- ‘Application for the post of ___,  post code ___, state Punjab’
• Send the complete application form by Registered post /Speed post at the following address – The Medical Superintendent, ESIC Model Hospital, Bharat Nagar, Ludhiana-141001 (Punjab).
• Documents to be attached along with the application form are -- date of birth certificate, all educational qualification (along with marks sheet), technical/professional qualification along with mark sheets, registration certificate with the concerned council wherever required, caste certificate wherever required, experience certificate wherever required, completion of internship certificate wherever required, disability certificate in case of candidate with disabilities (physically challenged), discharge certificate for ex-serviceman, copy of challan/online payment receipt in support of payment of fee wherever required.
