per the orders of the Mission Director, Rashtriya Gramin Swasthya Mission - Uttar
Pradesh (National Rural Health Mission Uttar Pradesh), Lucknow, the Office of
the Chief Medical Officer, Mathura invites applications for the posts of
Medical Officer, Staff Nurse and ANM on purely contract basis for various
projects of the NRHM.
Position
and vacancies:
- Medical Officer – M.B.B.S. (Women) – 2 posts in the Urban Health Post Project
- Staff Nurse – 2 posts in the Ashirvaad Baal Swasthya Guarantee Yojna and other projects
- ANM – 10 posts in the Ashirvaad Baal Swasthya Guarantee Yojna and other projects
Eligibility:
- Medical Officer – M.B.B.S. and registered with the U.P. Faculty.
- Staff Nurse – Diploma in J.N.M. and registered with the U.P. Medical Board
- ANM – Diploma in A.N.M and registered with the U.P. Medical Board
Age
limit: Between
21 to 65 years of age

- Application along with attested copies of all the relevant documents (including domicile certificate) and two self addressed `25 stamped envelopes should be sent by registered post to the following address – The Office of the Chief Medical Officer, Civil Lines, Mathura.
- Mention the post and concerned project name on the envelope containing the application form.
- The details to be included in the application are – passport size photograph, advertisement number, post applied for, name of the applicant, father’s/husband’s name, date of birth, caste, permanent address, address for correspondence, contact number, educational qualifications, work experience, domicile, declaration and signature.
- The date of interview is December 26, 2012 from 10:00 a.m. onwards at Achal Prashikshan Kendra Nabada, N.H. - 2, Mathura. No separate intimation will be sent for interview.
